Change of Authorized Representative Name

 

1.  Access the Authorized Representative’s Family Information Page.

2.  Click inside the name field and behind the name that is to be changed.

3.  Use the Backspace on your keyboard to delete the former name.

4.  Enter the new Name.

5.  Click the Save button. See Module 2: Figure 51 below.

When the name is changed and a save function is performed on the Family Information Page, HANDS will alert the user that the Representative’s Proof of ID is required. See Module 2: Figure 52 below.

The user will need to add the new Proof of Identity record and click the Save button.
